An oxygen cylinder caused an explosion on a Qantas plane last week, air safety investigators have confirmed.

The Boeing 747, carrying 346 passengers and 19 crew, was forced to make an emergency landing in the Philippines capital Manila after a hole was blasted in the fuselage.

The Australian Transport Safety Bureau's preliminary investigations have also found that some of the plane's oxygen masks did not work.
