A new report is calling for an urgent overhaul of the aged care system saying older people are struggling to access services.
The Aged Care Commissioner's study shows some people are getting stuck in hospital because there is no support for them at home.
It has laid out 20 recommendations to improve things, including better integration between public and private healthcare.
Commissioner Carolyn Cooper spoke to Corin Dann.
