The fatal crash of an Australian helicopter off Fiji in 2006 has been blamed on pilot error, but with a number of contributing factors.
The accident, which killed the pilot and a soldier, occurred as the aircraft was conducting exercises from the landing ship HMAS Kanimbla in international waters off Fiji.
An inquiry found there was a gradual adoption of approach profiles which on occasions exceeded the limits of the aircraft.
It says other factors include a can-do culture in their squadron, inadequate supervision, the pressures of preparing for operations, the re-location of the squadron and high operational tempo
