The inquiry was one of the Labour government's election promises.

The six member inquiry team was welcomed with a pōwhiri.

It will hear submissions from a range of groups including DHBs, the police and judges.

The panel will travel the country hearing from patients, staff, those in the community about problems and possible solutions to improve services for patients and their families.

People can give their views on the phone, in person or in writing.

Chair Ron Paterson was confident the inquiry would find better ways of doing things.
