Wellington Coroner Ian Smith has listed recommendations on diver safety in his findings into the death of a woman while diving.
Jacqueline Ngapera, 37, died after failing to resurface from a dive in December 2007 off the Wellington coast.
Her body was found on the seabed the following day.
A police dive squad member said Ms Ngapera and her diving buddy had an insufficient dive plan and she was not wearing a watch to monitor the time she had spent in the water.
The recommendations include that divers monitor their depth and time and have a plan if an emergency occurs.
